Care Coordinator – Domiciliary Care

Location: Oxford and surrounding areas 

Hours: 40 hours per week (full‑time)

Additional Requirements: Participation in on‑call emergency rota

Salary: Competitive, dependent on experience

About the Role

The Care Coordinator plays a central role in ensuring high‑quality, person‑centred care is delivered to clients across Oxford and the surrounding areas. This position requires a proactive, organised professional who can balance scheduling, compliance, communication, and crisis management with confidence and compassion.

You’ll be the operational heartbeat of the service — coordinating care packages, supporting care staff, liaising with families and professionals, and ensuring smooth day‑to‑day delivery of care.

Key Responsibilities
  • Develop and maintain effective rotas to ensure all care visits are covered in line with client needs and contractual requirements.
  • Respond promptly to changes in care packages, staff availability, and emergency situations.
  • Participate in the on‑call rota, providing out‑of‑hours support for urgent issues.
  • Conduct client assessments, reviews, and risk assessments in line with regulatory standards.
  • Maintain accurate, compliant records using digital care management systems.
  • Build strong relationships with clients, families, care staff, and external professionals.
  • Support recruitment, onboarding, and supervision of care staff.
  • Ensure all care delivery meets CQC standards and internal quality benchmarks.
  • Promote a culture of dignity, respect, and person‑centred care throughout the service.
Required Qualifications & Experience
  • Level 3 Diploma in Health & Social Care (or equivalent) — essential.
  • Working towards or willing to complete Level 4 Certificate in Principles of Leadership and Management for Adult Care — desirable.
  • Proven experience in a care coordination or senior care role within domiciliary care.
  • Strong understanding of CQC regulations and compliance requirements.
  • Confident using digital scheduling and care management systems.
  • Full UK driving licence and access to a vehicle.
